Please bring the following documents with you to every appointment:
- Chip card
- Vaccination card
- If applicable, routing slip (for employment and termination of employment)
- Employment examination certificate, if applicable
- Student ID card, if applicable
- Current laboratory results/reports of findings/ AHB reports, if applicable
